Smiling Nurse

Licensed Practical Nurse (LPN)

Valleyview Assisted living is searching for an LPN registered and licensed with the State of Minnesota. This role is an important position within our community, assisting in the oversight of our team of Health Aids and ensuring our resident's health and happiness.

The RN role consists of 40 hours/week Monday – Friday and on-call every other weekend.

Job Summary: The role of the LPN is to ensure that the health and safety of all residents are met through working with the residents and staff. The LPN will report to the RN and Director, ensuring that HHA’s are hired, or trained, to state-required competencies standards. The LPN will also assist the facility, ensuring compliance with state regulations and standards. Must be able to relate professionally with residents, staff, and family members, display a positive work attitude, and be able to work as a team with other staff members.

Responsibilities:

  • Work with RN to ensure that Valleyview of Jordan, LLC can meet the client’s needs according to the initial assessment.

  • Ensures residents maintain admission criteria, reassess, and recommend alternative placement if necessary.

  • Completes Assessments with the RN

  • Implements service agreements with all new residents. Reviews and revises service plans as needed at a minimum of once per year.

  • Ensures residents have up to date care plans, in conjunction with the RN

  • Monitors resident charts and clinical progress notes

  • Works with the RN to communicate to physicians and other personnel (county case workers, etc) any changes in the resident’s needs or conditions * Works with the RN to facilitate coordination of resident services with other service providers.

  • Works with the RN to ensure all physician orders are signed and implemented with-in 24 hours

  • Ensures that medication administration procedures are followed properly and discusses any needed corrections, errors, etc., with staff.

  • Communicates in resident’s log any information that staff need to be aware of.

  • Works with the RN in the documentation of the disposal of medication per procedure each week.

  • Performs supervisory visits 14 days after admission and at least every 62 days thereafter.

  • Works as a team player with all members of the resident's care team.

  • Works with Administrator and RN in providing training to all new employees as well as continued education for all staff and documents such training to ensure compliance with state, federal, and local regulations

  • Ensures that staff clearly understands and follows all resident care procedures

  • Monitors staff for accuracy of monthly resident blood pressures, weight and pulse as requested

  • Shares resident and family concerns with pertinent staff if appropriate.

  • Assists in the development of policies and procedures and implementation of them.

  • Attends staff meeting.

  • Works with Administrator to ensure emergency procedures are in place.


Cooking Eggs

Food and Beverage - Line Cook

Sets up and stocks food items and other necessary supplies. Prepares food items by cutting, chopping, mixing, and preparing sauces. Cooks food items by grilling, frying, sautéing, and other cooking methods to specified recipes and standards.
